All form of criticism, suggestions, and complaints are submitted by the public to an agency are a correction to improve the service many village offices in Indonesia have not bad a well identified and documented complaint service system which resulted in problems in providing complaint services to the community will be designed using a prototype model and mysql as the database on the sever wed it is expected will be easier for the public to make complaint reports this website application can used by the public to deliver online complaint services this application also make it easier for the village goverments to document and evaluate the village community complaint service system.
